Procedure Costs

CDT code reference with fees, Maryland Healthy Smiles coverage, and typical frequency limits. Fee equals Maryland Healthy Smiles pays plus patient pays. Most covered services — including fillings, prophy, exams, endo, and extractions — are paid in full by Medicaid, so the participant owes $0. Patient-pays is only > 0 for codes MHS doesn't cover (implants, fixed bridges, cast-metal partials, etc.) or when a service exceeds its frequency limit.

These numbers are a quick reference only — always verify the current fee, coverage, and frequency yourself before quoting a patient or submitting a claim. Frequency limits and pre-auth flags reflect Provider Manual V16 (effective 1/1/2025) and cover adult, child (<21), REM, and Former Foster Care (21–25) plans. Where limits differ, the adult value is shown first; child/REM variants follow in parentheses. "Kids only" means the code is not a covered benefit under the adult plans.
